Telephone pole and adjoining pipe are in the middle of a sidewalk and impairing sidewalk access. Please coordinate correction with utility company. Thank you.

A sidewalk and ramp were damaged during a water line repair at the Southwest corner of W. Drew and Morgan. Repairs were made, however the contract did not align the ramp base with the street. The street is higher than the ramp base which makes it difficult for those with mobility devices to traverse. It also causes mud and water to drain into the ramp "basin" making passage more difficult for all. Please request the contractor fix the ramp to align with the street as has been done in other situations with this predicament. Thank you.

Sidewalk was repaired after partial removal during public works project. However, an ADA compliant sidewalk ramp was not installed. Sidewalk is located on the Southwest corner of Willard and Converse. Please require contractor to install ramp as required. Thank you.

Sidewalk was damaged during water line contruction and groundfill under the sidewalk was removed during work. The sidewalk repair did not include replacing the groundfill to make the sidewalk level before replacing. Please require city contractor to fix sidewalk installation. This has occurred in other areas and is slowly turning the sidewalk grid into a roller coaster. Thank you.

A sidewalk and curb on the Southwest corner of Stanford and Welch was repaired after water line work, however the contractor did not install a sidewalk ramp as required. Please require contractor to fix. Thank you.

Sidewalk and underlying dirt removed as part of a water line repair project. Sidewalk was installed without replacing the dirt creating a slope along the sidewalk (in addition to the expected slope of the ramp). Please correct sidewalk slope and provide a level surface as it was before. Thank you.
